If collecting through your own bank, you will also end up collecting the $50 Membership fee that is due to Fidelis. This will be invoiced and collected with the October and April Invoices. Check the box to acknowledge.

If we are collecting your fees for you, you will have the choice of having them sent to you via check in October and April, or they can serve as a credit on your account to offset Annual Subscription Dues (charged with the October invoice) or any discount code redemption dues (charged in April invoice.)
